As part of your booking request, you’ll be asked to complete a short screening questionnaire. Your appointment will remain pending until I’ve reviewed your responses and confirmed that Spinal Energetics is appropriate for you.

Once your appointment is confirmed, you’ll receive a more detailed intake form to complete in preparation for your session.
